Follicular Decalvans and Lichen planopilaris phenotypic spectrum: a case series of biphasic clinical presentation and theories on pathogenesis

The authors present a case series of 13 patients with predominant Folliculitis Decalvans transforming to Lichen planopilaris- clinically as well as histopathologically both sequentially over the course of treatment and also in some case concomitantly at the same time in different locations on the scalp.
Abnormal HF inflammatory responses and collapse of HF immune privilege in response to microbiome dysbiosis was one of the possible theories hypothesized to explain the likely pathogenic mechanism that underlie this spectrum of phenotypic appearance.
Understanding of the underlying aetiology and potential overlap between these seemingly distinct conditions ought to help improve current treatment protocols for these biphasically presenting Primary Cicatricial Alopecias- in this case this would mean treating both the conditions separately and thoroughly for adequate management.
